Introducing Lakeside Healthcare Stamford

Thank you for taking part in our recent consultation about merging the patient lists at St Mary’s Medical Centre, Sheepmarket Surgery and The Little Surgery to create one practice in Stamford.

The consultation ended on 1 May 2017 and the feedback has been reviewed and analysed and discussed with NHS England and South Lincolnshire Clinical Commissioning Group.

We are pleased to advise that our application has been approved.  This means that we will now be able to merge our patient lists and work as one practice in Stamford.  Our new practice will be called Lakeside Healthcare Stamford.

Between now and when the official merger process is completed on 22 November 2017 there will be a period of operational change.

Phase 1

The Sheepmarket Surgery and St Mary’s Medical Centre will merge their clinical systems on 8 August 2017. Subject to this being successful, from 9 August 2017 patients will be registered with Lakeside Healthcare Stamford.  The Little Surgery will not be affected by this change.

Phase 2

On 21 September 2017, The Little Surgery will move over to the same clinical system as Lakeside Healthcare Stamford.

The Little Surgery building will close at 18:30pm on 21 September 2017 and the practice will relocate to the Sheepmarket Surgery site, reopening here at 08:00am on 22 September 2017.

Little Surgery patients will still be registered with The Little Surgery and will be able to access their services from both Lakeside Healthcare Stamford sites (Sheepmarket Surgery or St Mary’s Medical Centre).

Phase 3

On 22 November 2017, The Little Surgery will merge its clinical system with Lakeside Healthcare Stamford. Subject to this being successful, from 23 November 2017 these patients will also be registered with Lakeside Healthcare Stamford.

This will complete the merger process.  All Lakeside Healthcare Stamford patients will then access their services from the Sheepmarket Surgery or St Mary’s Medical Centre sites.

What does this mean for me?

Patients at The Little Surgery

  • The Little Surgery will continue to operate as it currently does until Dr John Fields retires at the end of August 2017.
  • On 1 September 2017 all patients will become registered with Dr Melanie Denton and will be seen either by Dr Denton or by another doctor from the Lakeside Healthcare Stamford team at The Little Surgery.
  • On 21 September 2017 The Little Surgery will change its clinical system to SystmOne.  This is the same clinical system as the rest of Lakeside Healthcare Stamford.  Access to current online services will end and Little Surgery patients will need to request new online services access details for the new system from 22 September 2017.
  • On 22 September 2017 The Little Surgery patients and its team will move to the Sheepmarket Surgery site.  You will no longer be able to access services from The Little Surgery building and will need to contact either the Sheepmarket Surgery site on 01780 473017 or the St Mary’s Medical Centre  site on 01780 764121.
  • You will be able to book appointments at either site and our Reception Teams will be able to access all available appointments.
  • We will continue to process repeat prescriptions and dispense medication (to eligible dispensing patients) for you from either site but we will assume that you will collect your prescriptions and medication from the Sheepmarket Surgery site unless you tell us otherwise.
  • If you need a blood test you will need to attend the Pathology Department at Stamford and Rutland Hospital.  Your GP or Nurse will give you a blood test request form first, or will leave it for you at Reception for you to collect, and will explain to you how the new system works.
  • We will launch a new Same Day service from 22 September 2017.  This service will be for patients who feel that they need to see a Doctor or Nurse the same day and will replace the current walk-in service at The Little Surgery.  The service will start with a telephone appointment with the Duty Nurse or Duty Doctor who will assess your condition and provide you with the care, advice and treatment that you need.
  • Unless you register with a different practice before 22 November 2017, you will become registered with Lakeside Healthcare Stamford.
  • To help balance our registered patient lists, we will change the registered GP for any patients previously registered with Dr Fields to a different male GP shortly after 22 November 2017.  You will still be able to see any of the GP’s in the Lakeside Healthcare Stamford team and will be able to change your registered GP to your preferred choice.
  • All members of the virtual patient group at The Little Surgery will be invited to join the new Lakeside Healthcare Stamford Patient Group. If you would like more information about our Patient Group or would like to join, please contact Reception.

We have tried to answer some of the questions we think you may have above and this information can also be found on our practice websites.  Combining our patient lists to create one practice will help us to develop the new and innovative services that will better support your future needs and we look forward to developing and sharing them with you.

If you have any specific questions that we have not answered our Practice Managers Emma Downs and Simon Stitson are here to help.  You can email them at emma.downs@nhs.net or simon.stitson@nhs.net.  In the meantime, thank you for your support and welcome to Lakeside Healthcare Stamford.

Alternatively, if you need any help or support with the changes, you can contact your local PALS service.  The Patient Advice and Liaison Service is free and completely confidential and can be contacted on 0845 602 4384 or by email at lhnt.lincspals@nhs.net.
